Method 1
Fill out the form, print it and mail it to the MDA membership contact, with your check made payable to MDA, INC.
Method 2
Fill out the form and submit it, and you will be granted a temporary membership contingent upon receipt of your check by the MDA membership chairman. During that time, you will be entitled to receive the MDA Newsline, attend any Board meetings occur during that time, and you will be eligible to enter in any upcoming clinics and programs.
Instructions for filling out the application form
Fill out the application form below by typing into the blocks and clicking on the blocks that show choices. When you are finished, click "Submit Form". If you want to clear the form before you submit it, click "Reset". After you submit the form, you will see a confirmation screen. If you do not see this screen, your application may not have been received. If that happens, fill out the form again and resubmit.
